  6. I feel unmotivated to do things that I used to like doing.
  7. I feel sad more days than not for no reason.
  8. I have trouble completing tasks that I need to complete.
  9. I leave many projects partly completed.
  10. I feel the need to check things when I know I have already checked them.
  11. I have annoying thoughts that I cannot stop myself from thinking about.
  12. I try to restrict my caloric intake for fear of being or becoming fat.
  13. I have struggled with getting to appointments in the morning because I stay up too late.
  14. I am unhappy in my significant emotional relationship.
